What to put on a therapy invoice
- Your name, credentials, license number, NPI and tax ID.
- The client’s name and date of birth.
- Each session with its date, length and CPT code (90837 for 53 minutes or more, 90834 for 38–52, 90791 for an intake assessment).
- The diagnosis code (ICD-10), if the client will claim from insurance.
- Fees charged separately, such as late cancellations, and payments received.
Superbills, privacy and good faith estimates
Clients who see you out of network can claim from their insurer with this invoice as a superbill, once it shows the session codes, diagnosis and your NPI. It contains health information, so send it through your client portal or secure email. Self-pay clients are entitled to a good faith estimate of expected charges before treatment under the No Surprises Act; use the estimate template for it. Late-cancellation fees usually aren’t reimbursed by insurers; list them separately.
Frequently asked questions
Should I invoice therapy clients per session or monthly?
Either. Many therapists charge at each session and send a monthly statement or superbill listing all sessions, as in this template.
Can I charge for missed sessions?
Yes, if your policy was agreed in writing beforehand. Show the fee as its own line with the date.
Is therapy subject to sales tax?
Health care services, including psychotherapy, are generally exempt from sales tax in the US.
